New Patient Information

Transferring from another office:

If you would like to establish your child(ren) at our office, please call  217-855-4969 to request the new patient questionnaire. Once the questionnaire has been reviewed you will hear back from our office via the e-mail you have provided to us. If you are transferring from another office, the process is quick and easy, our office staff will walk you through it.

Newborns:

For new parents to be, we recommend calling and getting the new patient questionnaire filled out PRIOR to delivery.  If this is not possible, you may call us and request a new patient questionnaire once your baby arrives.  Keep in mind, there is a short time lapse between filling out the forms and getting a response so we recommend doing this ahead of time if at all possible.  Newborns who come to us will be seen in the office within 2-5 days of discharge from the hospital.  There may be times when we are not accepting new patients, if we are currently full, you can be added to our waitlist.

Twilight Enhanced Access Costs:

Enhanced access care is a model of health care that gives the control back to the patient and provider without insurance dictating the care, tests, and time you may receive. With a smaller practice size, we can spend more time with you and your child and delve into issues that may not be addressed in the hurried environment of a traditional pediatric practice. 

Phone calls are answered personally and getting an appointment scheduled is quick and easy. In our practice you won’t sit in a crowded waiting room or speak to an answering service who you have never met; as you will always have a portal to message your pediatrician at any time.  Our tailored level of care and patient experience is significantly different then a traditional practice setting and it truly is an investment in your child's health and the experience in which you and they have in the office.

In order to provider continual coverage with online patient portal access to the providers for after-hours services and telehealth services, unscheduled urgent weekend visits, there is a monthly fee for each patient which covers the enhanced access provider, this is care that goes beyond tradition medical services.

  • Twilight Pediatrics provides immunizations for patients in the office in accordance to the national recommendations per the CDC and the American Academy of Pediatrics;  we do not make adjustments to the vaccine schedule and we do not accept any non vaccinated children.  For more information on vaccines you can visit: https://www.cdc.gov/vaccines/hcp/imz-schedules/child-adolescent-age.html

  • Please note that our new patient process takes a lot of variables into consideration when taking new patients - including age of patient, how many kids in the family, complexity of the patient, and our office availability to meet your child's needs.  We accept children based on what we can safely take on at this time, so there are times we are only accepting newborns or transfer patients (or neither), depending on our current patient load.

    If you are not accepted at the time of request, we will be happy to add you to our waiting list and will contact you when we have openings available.
